By checking, I give permission for my child(ren) to participate in the Hyper Nerf Knockout activity hosted by  Grace Baptist Church. I do hereby release Grace Baptist Church and its ministries, its Directors, Employees, Agents, and all Organizers and Helpers from any and all liability, claims, or demands for personal injury, sickness, or death.  I further understand that Grace Baptist Church does NOT provide health insurance coverage for my child.I understand that my child(ren) may be photographed or video recorded, either individually or with other participants or attendees, at such activities or events. I consent to my child(ren) being photographed or video recorded, and I consent to the use of any such photographs or video for any legitimate purpose by Grace Baptist Church and the organizers of the event.
